Detecting New Issues

[Under development]

  • Do not wait to be “updated” in a status meeting that there is an issue.

  • Issues need to trigger discussion and then action with little delay.

  • An issue is usually one of:

    • A metric is out of its expected range.

    • Something external occurs that changes assumptions made in a prior decision.

    • A new idea is proposed.

    • Conflict or dysfunction becomes apparent.

  • This event-based approach requires that,

    • There have been quality discussions about the current model of how the value stream works.

    • Metrics about the above have been defined.

    • People (including managers) are paying attention to the metrics.

    • People are not afraid to notify managers when an issue arises.
